The short version

  • Rheumatology guidelines strongly recommend exercise for knee osteoarthritis.
  • Stronger thighs and hips help the knee handle load.
  • Swelling, locking or giving way needs a clinician.

Get checked first if

  • The knee is hot, very swollen or you cannot bear weight
  • It locks, catches or gives way
  • Pain followed a twist or fall

A 15-minute knee-friendly routine

  1. Bike or walk 5 minutes.
  2. Wall sit at a comfortable depth: 3 × 20–30 s.
  3. Glute bridge: 2 × 12.
  4. Step-up on a low step: 2 × 8 each.
  5. Box squat or goblet squat to a comfortable box: 2 × 10.
  6. Calf raises: 2 × 12.

A common rule many physiotherapists use: pain up to about 3 out of 10 during exercise that settles by the next day is usually acceptable. More than that, ease off.

Is it bad if my knees go past my toes?

For most healthy people, no. Keep the whole foot down. With knee pain, use a depth that feels okay.
